Medical Accommodation/Leave Forms
If you need a medical accommodation or medical leave, the district will ask your doctor to fill out one of the following forms. We recommend taking the form with you to your medical appointment to avoid multiple trips to the doctor. It is normal for your doctor to charge you a fee to complete the form. Most doctors charge around $50.
Please note that your doctor is not obliged to give a diagnosis to the employer: only to say that you are under their care, they recommend a medical leave or accommodation and that you are following their treatment plan. If you're asking for an accommodation, the employer needs to know what your limitations are.
